Fill in the claim form and forward it to us along with the required documents.

CLAIM PROCESS

  1. Report the accident or theft to the Police immediately
  2. Report the accident to CIC as soon as possible.
  3. Do Not Accept liability in case of an accident
  4. Call CIC if you need to tow the vehicle from the scene of the accident (for comprehensive insurance only)
  5. Complete and return a claim form obtainable in all our offices and website.
  6. Attach the Police Abstract and a copy of the driving license of the person who was driving the vehicle at the time of the accident or loss.
  7. Arrange inspection of the damaged vehicle
  8. Cooperate with any appointed third party (Investigator or Assessor)

  2. Copy of the diving license for the person driving the vehicle during the accident
  3. Police Report Abstract

CLAIM PROCESS

  1. Report to the police immediately and obtain a police abstract
  2. Report the loss to CIC immediately through the given emergency or toll-free numbers
  3. Complete and return a claim form obtainable in all our offices and web site giving the replacement cost of lost property
  4. Attach a statement from a watchman/guard or any witnesses

CLAIM PROCESS

  1. Report the accident to CIC as soon as possible in writing.
  2. Complete and return a claim form obtainable in all our offices and web site
  3. Attach police Report abstract if it was a road traffic accident or assault
  4. Attach copy of death certificate if the accident was fatal
  5. Attach copies of pay slips for the previous 12 months before the accident

CLAIMS PROCESS

  1. Claims should be notified by the insureds to CIC offices through direct, email or telephone.
  2. Complete the claim form that is provided by the Claims officer.
  3. Submit all necessary documentation requested by CIC insurance.

REQUIRED DOCUMENTS

  1. Duly completed claim form.
  2. Copy of ID.
  3. Bank account details.

GROUP LIFE PRODUCT : GROUP FUNERAL

CLAIMS PROCESS

  1. Claims should be notified by the insureds to CIC offices through direct, email or telephone within six months.
  2. Complete the claim form.
  3. CIC will acknowledge receipt within 12 hrs.
  4. Submit all necessary documentation requested by CIC insurance.
  5. Claims discharge voucher will be issued after receiving all necessary documentation.
  6. CIC will issue the payment within 48 hrs after receiving discharge voucher.

  1. Duly completed claim form.
  2. Original death certificate/stamped letter from the group village headman.
  3. Copy of ID card.
  4. Doctors medical report.
  5. Police abstract in case of unnatural death.

GROUP LIFE PRODUCT : EMPLOYEE GROUP LIFE

CLAIMS PROCESS

  1. Claims should be notified by the insureds to CIC offices through direct, email or telephone within six months.
  2. Complete the claim form.
  3. CIC will acknowledge receipt within 5 working days.
  4. Submit all necessary documentation requested by CIC insurance.
  5. Claims discharge voucher will be issued after receiving all necessary documentation.
  6. CIC will issue the payment within 48 hrs after receiving discharge voucher.

CLAIMS PROCESS

  3. CIC will acknowledge receipt within 5 working days.
  4. Submit all necessary documentation requested by CIC insurance.
  5. Claims discharge voucher will be issued after receiving all necessary documentation.
  6. CIC will issue the payment within 48 hrs after receiving discharge voucher.

REQUIRED DOCUMENTS

  1. Duly completed claim form.
  2. Original death certificate.
  3. Copy of ID card for the principal and claimant.
  4. Doctors medical report in case of Total permanent Disability or Critical illness.
  5. Police abstract in case of unnatural death.
  6. Account statement for the disbursed/claimed loan.
  7.  Sufficient documentary evidence in case of retrenchment.

Death Claim Requirement(For Family Life Plan/Last Expense Claims, only requirements 1-6 are applicable)

  1. Copy of ID and Beneficiary (Original & Copy)
  2. Copy of ID of Policy Holder (Original & Copy)
  3. Burial Permit (Original & Copy)
  4. Certificate of Identity
  5. Certificate of Attending Physician
  6. Police Abstract if an accident is involved (Original & copy)
  7. Copy of ATM card of the claimant

  9. The Original Policy Document (If lost Provide affidavit)
